5 minutes before your appointment:
- Click the link on this page OR in your reminder email
- You’ll enter a private virtual waiting room
- Your ND will join at your scheduled time

Step 3. Payment
Your credit card is securely stored at the time of booking and will be automatically charged after your appointment is complete.

Accessing your Visit Summary
Quick steps:
- Click “Visit Summary” icon
- Select your visit date
- View supplements and recommendations made by your ND
Document Sharing and Uploading
To view shared files:
Click “Documents” icon → View or download
To upload your files:
- Click “Documents” icon
- Click blue “Upload” button
- Select file from your device
- Click “UPLOAD and SHARE” (not just Upload)
- Choose “CARE MEMBER” from dropdown
- Click “Share”
Accessing your Lab Orders and Results
To get your lab requisition:
- Click “Labs” → “Orders”
- Download and [print] the requisition
- Take to the lab specified on your requisition
⚠️ Important: Labs require printed requisitions (no electronic versions)
To view lab results after they have been reviewed at your appointment:
- Click “Labs” → “Results”
- Select date to view or download

How it works:
Submit your request: Complete the form below with the supplements you need.
We’ll send an estimate: Our team will review your request and email you an estimate to approve.
Pick up or delivery: Once approved, pick up at the clinic or we’ll ship your order directly to you.
Important: Refills are available only to patients who have had an appointment within the last 6 months. If it has been longer, please book a check-in visit first.
Common Questions
Q: How do I request supplement refills?
Click the “Supplement Refills” button at the top of this page to complete the refill form.
Once submitted, you’ll receive an estimate for your order. After you approve the estimate, your supplements will be prepared and shipped to you or ready for pickup at the clinic.
For questions about your supplement protocol or dosing changes, please book a follow-up appointment.
Can I get a receipt for my insurance?
Sales receipts will be emailed to the email address we have on file within 24-48 hours after your appointment. You can then submit your receipt for health insurance coverage, if applicable.
What type of appointment should I book?
We know it can be confusing to know where to start—so here’s a quick guide:
New to our clinic?
Book an Initial Naturopathic Visit (50 mins). This is a deep-dive into your health story, current concerns, and goals. We’ll start building your personalized treatment plan at this visit. Patients with complex health concerns are encouraged to book a Complex Health Concerns visit for 80 minutes to ensure sufficient time to review your concerns.
Returning patient?
Book a Follow-Up Visit. We offer different lengths depending on what you need:
25 minutes is great for quick check-ins, reviewing lab results, or making small adjustments.
Extended follow up visits for 50 minutes allows time for more in-depth support, like addressing new concerns or diving deeper into treatment.

After the visit, how do I access the details of my visit?
Your Naturopathic Doctor will create a Treatment Plan Summary, which you will be able to access through your patient portal at phr.charmtracker.com after your visit.
What happens if the internet connection is lost during the visit?
Your Naturopathic Doctor will stop the video consultation and call the phone number on file to complete your visit.
